REPORTS TO: PRODUCTION MANAGER at DAC, DACWORKS Lead Supervisor

DACMART RETAIL STORE MANAGER for job coaches working out of that site

STATUS: REGULAR, FULL TIME, HOURLY

MISSION: Dedicated to providing exceptional supports to empower individuals with disabilities to achieve success.

POSITION SUMMARY:

The Job Coach is responsible for carrying out the employment needs that have been identified through the person centered planning process for persons who have chosen services through Aitkin County DAC Job Coach provides a wide range of support: identifying client’s abilities, offering job readiness training, helping clients develop skills, providing on the job support and assistance as needed to help people with disabilities maintain meaningful and enjoyable employment. They are an integral DAC team member guided in a culture of positive purposeful driven work by encouraging ownership, utilizing staff and person served strengths and engaging team effort in all we do.

ESSENTIAL RESPONSIBITIES:

  • Assists the team in establishing and carrying out person centered work plans for the individuals served at DAC, DACMART, DACWORKS.
  • Responsible for the day to day directions given to the persons served while on a job site or while working in house at DAC, DACMART or DACWORKS. Their primary responsibility is to teach life and work skills to promote independence and success in the person’s chosen field of employment.
  • Documents all relevant information regarding goals, behaviors and accomplishments based on the individual person center plans. Completes daily documentation with the person’s residence and or guardians.
  • Responsible for tracking and reporting work tasks and hours for person’s served.
  • Transports person’s served to and from DAC, DACMART or DACWORKS in DAC owned vehicles.
  • Displays compassion and patience as each person served will require different one-one coaching to achieve successful employment.
  • Assists with planning and implementation of full campus and community attended events.
  • Responsible for completing orientation and annual training on time as assigned.
  • Responsible for attending bi-annual all staff meetings.

NON-ESSENTIAL RESPONSIBILITES:

  • Positive Ambassador representing Aitkin County DAC
  • Assists in keeping the DAC/DACMART/DACWORKS in clean, orderly manner
  • Supports team members in a positive manner

SKILLS AND KNOWLEDGE:

MINIMUM:

  • Background or training with developmentally disabled or seniors preferred
  • HS Diploma or GED
  • Ability to read, write and speak English at a level that meets performance requirements
  • Ability to work independently with minimal instruction and make independent decisions when circumstances warrant such action
  • Ability to meet licensing requirements of the state and applicable county, clear Criminal background check and completed fingerprinting
  • Ability to maintain good employee relations and morale
  • Willingness to take initiative and adapt to circumstances
  • Ability to follow daily routines while allowing for flexibility and planning creative alternatives
  • Ability to implement any therapeutic interventions as required
  • Ability to work in a variety of settings and with a variety of level of personal care needs without direct supervision
  • Ability to effectively use a computer and software, calculator, household appliances, smoke alarms, etc. with training
  • Ability to accept and incorporate new methods into existing practices
  • Valid Driver’s License is required with minimal violations (see MVR guidelines from Selective)
  • CPR and First Aid must be completed within 6 months of employment

WORKING CONDITIONS:

  • Is subject to frequent interruptions, imposed deadlines and frequent problem solving activities
  • May be subject to hostile and emotionally upset individuals served, staff, family members, visitors and personnel from other agencies
  • May be exposed to a variety of conditions, infectious diseases, odors, dust, etc. throughout the working hours
  • Works in a variety of settings inside and outside the program site and throughout the community
  • May be subject to physically aggressive individuals served
  • Is subject to a dress code of business casual dress during working hours, close toed shoes and proper steel toed boots on yard chore service crews

PHYSICAL DEMANDS

  • Sits, stands, walks, bends and moves intermittently during working hours
  • Lifting/transferring requirements vary based on assigned caseload
  • Ability to master the use of all physical intervention and assistance techniques, necessary to meet the needs of assigned individual with disabilities
  • Must possess sight/hearing senses or use prosthetic services that will enable these senses to function adequately
  • Ability to move intermittently throughout the work day
  • Possess good health and demonstrate emotional stability

STANDARDS OF PERFORMANCE:

1. Administrative Support Functions:

  • Assists with assuring the smooth day-to–day operation of the DAC/DACMART
  • Assists Executive Director and direct Work Supervisor as assigned
  • Tracks and documents person’s served work tasks and hours worked
  • Responsible for communicating program, work, health needs or accomplishments to team and person served home
  • Completes service person’s incident, injury and behavior reports as needed per policy

2. Care Need Functions:

  • Assist person served with daily living skills as stated in their CSSP addendum

3. Job Coach Functions:

  • Helps the person served understand their job duties and employer policies
  • Helps the person served build a maintain positive working relationships with co-workers and potential customers
  • Helps the person served develop strong work skills
  • Helps the person served understand how life outside of work may affect their performance at work
  • Responsible for understanding the person’s served person centered planning~ CSSP, CSSP addendum, IAPP and self-management assessment
  • Responsible for carrying out and documentation of goals, behaviors and accomplishments related to the person’s plan
  • Provides guidance and direction to person served while they are performing work and or activity choices

4. Training Functions:

  • Attends Bi-annual all staff meeting
  • Maintains proper education training to fulfill the direct care professional role as required on time
  • Understands basic knowledge regarding the impact of the Olmstead law, Workforce Innovation and Opportunities Act and Positive Supports Rule

5. Confidentiality:

  • Maintains a clean office area free of visible confidential information
  • Responsible for shredding confidential material as need
  • Handles and communicates discreetly and appropriately all confidential information guided by HIPPA and DAC policy

6. Collaboration:

  • Maintains positive relationships with service person team members, vendors and employment partners
  • Supportive team member communicating positively
  • Assists the person’s served as an advocate to help them with personal, social, care and work needs

7. DACMART Retail Store Functions:

  • Assists with determining quality in the store assisting persons served to sort, hang and price items
  • Assists with cash register and functions related to receipts and deposits
  • Assist in ordering supplies for the store
  • Assists with the rotation of items in store

8. DACWORKS:

  • Assists person served in woodworking shop
  • Assists with cash register and functions related to receipts and deposits
  • Assist in ordering supplies for the store
  • Assists with the rotation of items in store
  • Assists person served in vermiculture farming
  • Assists with events and community integration activities
