Job Summary
The Transitional Case Manager provides, person-centered case management services to individuals and families that resides in or transitioning from Permanent Supportive Housing (PSH) to independent housing. The role focuses on helping participants stabilize income, increase natural supports, improve daily living skills, and successfully move into and maintain long-term housing with/without ongoing rental subsidies.
The Case Manager assists participants in developing individualized transition plans, coordinating community resources, and ensuring continued stability during and after the move-out process.
Duties
- Conduct comprehensive strength-based assessments to determine readiness to transition from PSH.
- Develop Individualized Transition Plans (ITPs) with measurable goals that support stability and independence.
- Provide housing navigation, including assistance with housing search, landlord engagement, and apartment applications.
- Encourage self-advocacy and independence while respecting participant choice.
- Support participants in obtaining and maintaining income (employment, SSI/SSDI, benefits, or other income sources).
- Facilitate connections to mainstream and natural community supports (e.g., family, churches, clubs, community centers).
- Assist with budgeting, financial planning, and household management skills.
- Coordinate move-out assistance including referrals to moving services, utility setup, household furnishings, and transportation.
- Provide brief post-move stabilization support (typically 3–6 months) to prevent housing crises.
- Maintain accurate, timely documentation in HMIS and agency case notes.
- Ensure compliance with all HUD, program, and agency policies and procedures.
- Participate in team meetings, trainings, community partnerships, and case consultations.
Skills
- Strong understanding of Housing First, Harm Reduction, and Trauma-Informed Care.
- Working knowledge of housing programs, tenant rights, and landlord mediation.
- Ability to build rapport with participants from diverse backgrounds.
- Strong organizational skills and ability to manage a flexible caseload.
- Ability to work independently in the field and maintain safety protocols.
- Effective written and verbal communication skills.
- Proficiency in Microsoft Office, HMIS, and electronic documentation systems.
Minimum Qualifications
- Associate’s or Bachelor’s degree in Social Work, Human Services, Psychology, or related field (or equivalent relevant experience).
- At least 1 year of case management experience, preferably in homeless services, supportive housing, mental health, or substance use programs.
- Must have a valid driver’s license and reliable transportation.
Preferred Qualifications
- Experience working with individuals who have mental health, substance use, or chronic health conditions.
- Familiarity with HUD housing programs and community resource systems.
- At least 1 year of case management experience, preferably in homeless services, supportive housing, mental health, or substance use programs.
- Must have a valid driver’s license and reliable transportation.
Job Type: Full-time
Pay: $30,000.00 - $35,000.00 per year
