Aging Life Care Manager Job Description
Qualifications & Experience:
- Active LMSW, LCSW-C license, or equivalent, or be a Master’s degree-level gerontologist; CCM certification a plus
- Experience in geriatrics, dementia, home health, mental health, or hospice care is required
- You need to be a self-starter, able to work independently and communicate well with clients, families, and other professionals
- This position requires making in-person visits to our clients in the DC Metro area
Duties & Responsibilities:
- Performs the initial assessment for clients.
- Accompanies clients to medical and other appointments.
- Makes regular client visits to implement care plans and monitor status. The frequency of visits (weekly, biweekly, monthly) is to be determined in consultation with the family and reviewed with the leadership team as needed.
- Writes client care plans and submits them to the leadership team for review.
- Prepares monthly written client summaries.
- Conducts ongoing communication with family and other service providers (includes supportive counseling for client and family). Communicates regularly with family members by phone or email, after each visit, or less often as preferred by the family.
- Consults with Director and other staff regarding urgent issues such as a change in health status; the need for increased services; crisis intervention; problems with communication with family members or other health care providers; and ethical issues.
- Develops working relationships with leaders in the community by participating in professional networking activities, and sharing information about our services
- Provides occasional weekend on-call support to respond to urgent client needs or emergencies
