Summary
We are seeking a motivated and dynamic Home Health Coordinator to join our team and drive referral growth throughout the Bay Area. The Home Health Coordinator is responsible for building relationships with referral sources, physicians, case managers, fiduciaries, and community partners to generate home care referrals. This role combines sales strategy, relationship management, and healthcare knowledge to support patient access to care and ensure our agency remains a trusted provider in the community.
Key Responsibilities
- Develop and maintain strong relationships with referral sources (physicians, discharge planners, SNFs, fiduciaries, community organizations).
- Identify and close new referral opportunities through consistent outreach, cold calling, and follow-up.
- Educate providers, patients, and families on agency services, coverage, and care options.
- Monitor community, customer, and patient perceptions to position the agency as a high-quality provider.
- Provide feedback to leadership regarding market trends, competitor activities, and strategic opportunities.
Qualifications
- Bachelor’s degree in Marketing, Business Administration, or related field
- At least 1 year of sales experience preferred.
- Proven sales success (cold calling, closing, meeting/exceeding goals).
- Strong motivation and achievement drive (competitive, goal-oriented).
- Excellent planning, initiative, and teamwork skills.
- Exceptional interpersonal skills: assertive, diplomatic, extroverted, and cooperative.
- Inner strengths: confident, patient, and able to maintain a relaxed, professional style under pressure.
- Knowledge of home care, home health, hospice, or skilled nursing preferred.
- Must have valid driver’s license and reliable transportation.
