What are the responsibilities and job description for the HCBS Case Manager position at Ageless Healthcare?
Ageless Healthcare has a Case Manager position available! We are a homecare agency! You must have experience being a case manager. Be somewhat familiar with the LT-PCS, EPSDT, CCW, NOW, and SIL program rules and regulations. You must have great organizational habits, willing to be a networker, and work well with others. Be computer literate. Experience in Microsoft Word and Excel is necessary.
Be available to be on call at anytime for call ins with direct staff. Must be willing to provide in home care in related instances. Applicant must have their own transportation. Position will be full time. Interviews are being conducted immediately. Please submit a resume to this email.
