What are the responsibilities and job description for the Case Manager - Chittenden County position at Age Well?
We’re looking for a Case Manager to join our team!
Our Case Managers assess needs and coordinate and monitor community services and benefits for people over 60 to promote health, independence, social and economic wellbeing. This is a field position of some complexity, with particular attention paid to the most needy and at-risk individuals in the community at large. This position is also expected to work in the office on a regular basis. Mileage reimbursement provided.
We serve clients Monday thru Friday, 8:30am-4:30pm. No on-call nights or weekends.
The successful candidate will have a Bachelor’s Degree and some experience is preferred.
