Duties would include providing medical services to patients. Completing in home assessments and intakes. Assist with admissions. Complete 90 day follow-up care. Ensure the patient care plan is being followed. Complete required notes and documentation. Educate patients and families on care, treatments and answer questions.

Must be a licensed LPN. Experience in a home health care setting is required.
