Agave Hospice and Palliative Care is currently seeking a dynamic Intake Coordinator to provide exceptional care to patient’s, families and staff enrolled in our Hospice & Palliative Care Program. Position summary
Coordinates all aspects of patient care including admissions, discharges, patient assessments, care plans, etc. with patients, patients’ families, staff and administration by performing the following duties.
- Care coordination with family and providers
- Complete/Review admission paperwork
- Verify medical insurance and request authorizations
- Respond to phone, fax, and email inquiries
- Request medical records, update charts
- Ensure the highest level of customer service and quality patient care
