Basic Functions & Responsibilities:

The Parent-Child Assistance Program (PCAP) is a 3-year intervention providing direct outreach for pregnant and parenting women and birthing people. PCAP provides tailored, home-based, structured case management when at-risk alcohol or other substance use has occurred. Based in relational and harm reduction theory, PCAP provides trauma-informed, hope centered and goal-oriented support to clients to build their autonomy, establish a strong community support network, and reduce the incidence of future substance exposed pregnancies. Kitsap PCAP is one site of a statewide network of PCAP programs evaluated by the University of Washington’s Addiction, Alcohol & Drug Institute.


Specific Responsibilities:

· Perform case management activities according to PCAP policies and procedures, in a highly confidential and professional manner

· Provide supportive case management for a caseload of 15-20 clients

· Conduct home and/or community-based visits approximately twice per month, according to client need

· Assist clients in setting goals based on their individual needs assessments, identifying steps to meet their goals and monitoring progress

· Work collaboratively with other community providers and agencies whose services are relevant to client needs

· Provide outreach services to engage and connect clients with community agencies that will meet the client’s needs and goals

· Coordinate scheduling and attend case consultations and appointments with community providers with whom the client is involved. Maintain regular contact with the client’s network of service providers

· Provide outreach to locate missing clients and re-engage them with the program

· Collect client-related data that is often extremely sensitive and confidential, according to PCAP policies and procedures, and input into REDCap, an online data entry program

· Enter data and documentation accurately and according to established protocols and deadlines

· Attend and participate in weekly or bi-weekly individual supervision with site clinical supervisor to discuss client needs, goals, and issues

· Participate in weekly staff/ business meetings

· Complete all University of Washington PCAP training requirements upon hire and other training courses as required

· Obtain and maintain current CPR, Infant CPR, First Aid Certification and participate in annual Blood Born Pathogen training

· Contribute to report writing, account management and community presentations as needed

· Other duties as assigned


Requirements:

· 4-year B.A. or B.S. degree in a related field and at least 3 years of community-based experience working with vulnerable populations. Verification that a B.A. or B.S. degree has been achieved is required

· Demonstrated ability to effectively engage with mothers/birthing people and their families

· Experience with issues or on projects working with perinatal substance use or an equivalent combination of education and work experience strongly preferred

· Knowledge of substance use and its effects on individuals, families and communities is required

· Ability to maintain a positive relationship with clients, regardless of where the client is regarding substance use, and the conviction that substance use is a treatable illness and constitutes a public health challenge is preferred

· Strong written and verbal communication skills

· Ability to engage in conversations involving extremely sensitive & confidential issues

· Demonstrated ability to conduct themselves ethically & professionally, adhering to the agency Code of Ethics is always necessary

· Ability to work both with a team and independently

· Knowledge of community/recovery resources

· Ability to use an online data entry platform

· Strong organizational and time management skills

· Must have valid WA state driver’s license, a clean driving record and be able to travel on a daily basis. (Company vehicles are used)

· Ability to pass required background check

· Ability to meet the physical and training requirements of the position

· Applicants understand and adhere to state & federal confidentiality regulations

· Must sign an oath of permanent confidentiality covering all client related information
