Part-Time or PRN OT Home Health / Occupational Therapist

Job Summary:
- Assume responsibility for the ongoing assessment and development of individualized care plans.
- Provide ordered occupational therapy services and patient education.
- Evaluate functional status and home environment for hazards or barriers to independent living.
- Develop treatment programs and establish goals for improved function.
- Maintain appropriate clinical records and reports.
- Participate in quality assessment and performance improvement programs.
- Identify patient and family needs for additional home health services and make appropriate referrals.
- Instruct and supervise Certified Occupational Therapy Assistants.
Job Requirements:
- Must be licensed in the location where the Home Health Agency (HHA) is located.
- Graduated from an OT program accredited by ACOTE of AOTA.
- Eligible for or have taken the OT certification examination administered by NBCOT.
- Two years of clinical experience as an occupational therapist.
- Demonstrate good verbal and written communication, and organization skills.
- Possess and maintain current CPR Certification.
- Must be a licensed driver with a reliable insured automobile.
