Per Diem Social Worker Hospice

Section 1: Overview
- Implement standards of care for medical social work services
- Assess the psychosocial status of patients and families/caregivers
- Carry out social evaluations and provide counseling as needed
- Maintain clinical records and provide information and referral services
Section 2: Qualifications
- Graduate of a master's program in social work accredited by the Council on Social Work Education
- Minimum of one year's experience in health care, hospice experience preferred
- Good verbal and written communication skills
- Possess and maintain current CPR Certification
