Hospice Nurse Practitioner
Location: Clackamas, OR
Schedule: Full-Time | Monday–Friday | In-Office
Pay: $140,000/yr

Agape Hospice NW is dedicated to providing compassionate, high-quality hospice and palliative care to patients and their families. We are looking for an experienced Nurse Practitioner to join our team and support our clinical operations and patient care.
Job Summary:
The Hospice Nurse Practitioner will provide advanced clinical support to our hospice team, assisting with patient assessments, symptom management, care planning, and coordination of care. This is an in-office position working closely with our clinical team, physicians, and leadership.
Key Responsibilities:
- Provide clinical support and guidance to the hospice care team
- Conduct patient assessments and assist with diagnosis and treatment
- Develop and update patient care plans
- Manage symptoms and recommend appropriate interventions to promote comfort and quality of life
- Collaborate with physicians and the interdisciplinary team to coordinate patient care
- Review patient status, medications, and clinical needs
- Provide clinical guidance to nurses and other members of the care team
- Assist with hospice and palliative care consultations as needed
- Complete accurate and timely clinical documentation
- Participate in team meetings and support quality and compliance initiatives
Qualifications:
- Valid Nurse Practitioner license in the state of Oregon
- Minimum 2 years of experience in hospice, palliative care, geriatrics, or a related field
- Hospice and Palliative Care certification preferred
- Strong clinical assessment and critical-thinking skills
- Excellent communication and interpersonal skills
- Strong understanding of hospice philosophy and end-of-life care
- Ability to work collaboratively with an interdisciplinary team
- Strong organizational and documentation skills
