Key Responsibilities
The LPN Care Coordinator works closely with the clinical and administrative teams to support patient care coordination and intake processes.
Core responsibilities include:
- Coordinating patient referrals, admissions, and transitions of care
- Communicating with hospitals, facilities, providers, patients, and families
- Reviewing referral documentation and ensuring completeness and accuracy
- Supporting eligibility review and coordination with RN/clinical leadership
- Assisting with scheduling admissions and coordinating field staff
- Monitoring patient status updates and communicating changes to the care team
- Maintaining accurate and timely documentation in EMR systems
- Supporting interdisciplinary team communication and care planning
- Assisting with compliance, quality assurance, and regulatory requirements

Qualifications
- Active Oregon Licensed Practical Nurse (LPN) license in good standing
- Minimum 2 year of nursing experience required
- Experience in hospice, home health, case management, or care coordination preferred
- Strong computer skills and experience with EMR systems
- Excellent communication and organizational skills
- Ability to multitask and prioritize in a fast-paced environment
- Valid driver’s license and reliable transportation
- Comfortable working onsite with occasional flexibility to enter the field as needed
Physical Requirements / Working Conditions
- Primarily sedentary, office-based work with extended periods of sitting
- Frequent use of computer, phone, and office equipment
- Occasional standing, walking, bending, and light lifting (up to 20–25 lbs)
- May require occasional local travel for training or meetings
- Fast-paced environment requiring multitasking and attention to detail
