Job Description summary
The Business Development is responsible for building and strengthening community partnerships, increasing awareness of hospice services, and generating qualified referrals to support the growth and mission of Agape Hospice Inc. This role blends relationship-building, education, marketing, and care coordination, serving as a trusted representative and advocate within the healthcare community.
This position plays a critical role in driving census growth, supporting smooth patient transitions, and promoting compassionate, patient-centered end-of-life care.
Essential Job Functions/Responsibilities
1. Develop and maintain strong referral partnerships with Hospitals, physicians, and clinics, Skilled nursing facilities (SNFs) and assisted living communities, Case managers, social workers, and discharge planners. Actively generate hospice referrals aligned with growth targets. Maintain consistent in-person presence (“boots on the ground”) in the community Serve. as the face of Agape Hospice
2. Educate partners and community members on: Hospice eligibility criteria, Levels of care, Medicare and insurance hospice benefits, Differences between palliative and hospice care. Conduct: In-services, Facility presentations and Community outreach events. Address misconceptions and promote earlier, appropriate hospice referrals
3. Represent Agape Hospice with professionalism, compassion, and integrity. Promote hospice services using respectful, empathetic, and educational messaging. Assist with community engagement activities and brand visibility initiatives.
4. Collaborate closely with clinical and operational teams including admissions, intake, and nursing. Ensure timely, seamless patient transitions from referral to admission. Communicate effectively with patients, families, and referral partners throughout the process.
5. Track and report referral activity, conversion rates, and outreach efforts. Maintain accurate documentation of activities and community engagement. Ensure compliance with HIPAA, Medicare regulations, and all applicable hospice guidelines
Pay: $20.50 - $24.50 per hour
