Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor and observe patients’ health status, documenting vital signs and changes in condition accurately using Electronic Medical Records (EMR) or Electronic Health Records (EHR) systems
- Administer medications following prescribed instructions while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards
- Support clients with specialized care needs such as Alzheimer's disease, dementia, memory care, and end-of-life care with patience and compassion
- Operate patient transfer methods safely using equipment like Hoyer lifts or other patient transfer techniques to prevent injury
- Provide light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment
- Communicate effectively with clients and family members to ensure personalized care plans are followed and needs are met
Experience
- Prior experience working with individuals with developmental disabilities, geriatrics, or in assisted living environments is highly preferred
- Demonstrated knowledge of medical terminology, catheter care, wound management, and basic life support (BLS) or CPR certification required
- Experience working within home health agencies or providing hospice or palliative care is advantageous
- Familiarity with home health care assistance tools such as Hoyer lifts and patient transfer methods is desirable
- Proven ability to work independently while maintaining excellent patient communication skills and respecting patient confidentiality under HIPAA guidelines
- Previous roles involving long-term care or live-in employment experience will be considered a strong asset
- Ability to perform heavy lifting safely when necessary to assist clients with mobility or transfers
