Overview
Agape Comfort Care Solutions is a luxury In-home care provider dedicated to delivering compassionate and comprehensive care to individuals in need. We are seeking a Registered Nurse - Case Manager(PRN) to join our team. This role is essential in overseeing and coordinating in-home patient care, ensuring the highest standards of service while supporting clients and their families.
This position offers flexible, as-needed scheduling (PRN), allowing you to create a schedule that works for you while making a meaningful impact in the lives of those we serve. WE HAVE AN URGENT NEED FOR 4 CASES THIS WEEK. START ASAP! College Park, , Decatur, Barnesville & Jackson, GA.

Reports To: Chief Executive Officer (CEO)
Risk of Exposure to Bloodborne Pathogens: Moderate (Standard precautions significantly reduce risk.)
Responsibilities
✔ Comprehensive Client Assessment: Conduct initial and ongoing assessments of clients, gathering medical history, evaluating current needs, and developing individualized care plans tailored to their specific conditions.
✔ Care Plan Oversight: Create, implement, and regularly update care plans in coordination with clients, families, and interdisciplinary healthcare teams to ensure quality and continuity of care.
✔ Supervisory Visits: Conduct scheduled supervisory visits to client homes to assess caregiver performance, verify adherence to care plans, and address any client concerns or adjustments needed.
✔ Client & Family Education: Provide guidance and education to clients and families on managing health conditions, medications, and self-care techniques to promote independence and wellness.
✔ Regulatory Compliance & Documentation: Maintain accurate and timely documentation of client assessments, care plans, supervisory visits, and required reports in compliance with Georgia state regulations, HIPAA, and agency policies.
✔ Quality & Safety Monitoring: Evaluate the effectiveness of services provided, ensuring that caregiver interventions align with client needs and best practices.
✔ Interdisciplinary Coordination: Collaborate with physicians, therapists, social workers, and other healthcare professionals to ensure clients receive comprehensive, coordinated care.
✔ Advocacy & Support: Serve as an advocate for clients, ensuring they receive appropriate home care services and access to necessary community resources.
✔ Quality Assurance & Compliance: Participate in quality improvement initiatives and compliance audits to uphold high standards of patient care.
✔ Additional Duties: Perform other related duties as assigned by the Chief Executive Officer.
Qualifications
✔ Licensure: Active, unrestricted RN license in the state of Georgia.✔ Experience: Minimum of 2 years of clinical nursing experience, preferably in home health, hospice, case management, or a related field.✔ Knowledge: Strong understanding of Georgia state regulations governing home care services, as well as knowledge of disease processes, medications, and medical terminology.✔ Skills:
- Excellent assessment, communication, and interpersonal skills
- Strong organizational and time management abilities, with the ability to prioritize tasks and meet deadlines
- Ability to train and supervise caregivers (PCAs, CNAs, HHAs) and ensure compliance with care plans
✔ Other Requirements:
- Valid driver’s license and reliable transportation (Required for client home visits)
- Willingness to travel within the service area to provide supervisory visits and client assessments
- Must not be listed on the Office of Inspector General (OIG) Exclusion List or any state/federal disqualification lists prohibiting employment in healthcare services
