Agape Treatment Center, A top workplace three years running, is currently seeking a Primary Therapist for our facility in Wilton Manors, FL!The Primary Therapist is responsible for performing a wide variety of patient care activities as directed by the Clinical Director. He/she will be responsible for patient caseload and will provide group, individual, and family therapy. The Primary Therapist will conduct psychosocial and bio-psychosocial assessments. He/She will collaborate with the Treatment Team to formulate the Master Treatment Plan and assist in coordinating discharge planning. The Primary Therapist has primary responsibility for maintaining communication with the family, referral source and other service providers as neededSpecific Areas of Responsibility:Review assessments from referring facilities and note problems/issues that require further attention Contact referral source and other key individuals to gather additional information as necessary and maintain contactwith referral source and other key individuals throughout patient’s treatmentGather comprehensive diagnostic information through psychosocial assessment interview then develop anIntegrated Summary, bringing together information from all assessments to identify problem areas that may affecttreatment and/or need to be addressed in treatment planningDevelop a comprehensive problem list that reflects patient’s individual issues as described in assessments and theIntegrated Summary and present the psychosocial and diagnostic information to treatment team in a clear, concisemannerDuring first session with patient, review initial treatment plan, including case management issues and be sure thatpatient and staff assignments and responsibilities are understood and in progressFormulate a comprehensive treatment plan with patient that effectively addresses patient issues has clearly statedtreatment goals and objectives that are measurable and can be completed within established time framesParticipate effectively in multidisciplinary treatment plan reviews, demonstrating adequate preparation andunderstanding of patient issues, progress, strengths and weaknesses, areas of resistance, positive and negativestrategies, and use good clinical judgment in moving patient through treatment levels.Develop interventions that will accomplish goals and objectives and state the planned frequency of interventionsMaintain clinical records according to program policies and those of licensing and accrediting agencies, documentthe patient’s progress accurately as it relates to treatment goals and objectives, ensure that documentation is timely,consistent, clear, and articulate and keep clinical records always secure.Continue treatment planning process throughout patient’s stay in treatment, opening new problems and developingor revising treatment plans as necessaryLead and co-facilitate group therapy sessions in a manner that creates an atmosphere in which self- disclosure isfacilitated, helps patients gain group process skills, and helps patients gain insight into problems and utilizesexperiential techniques effectivelyComplete activity and group plans for the Clinical Director including supplies utilized and/or neededRequirements:Masters Degree RequiredWorking toward LCSW, LMFT, LMHC preferredA minimum of 1 year experience in substance abusepreferred.
