Outreach Specialist

Job Summary: The Outreach Specialist is responsible for conducting general prevention outreach to promote sexual health, reduce stigma, and improve engagement among populations in need and targeted HIV outreach, education, and linkage activities in accordance with Ryan White HIV/AIDS Program standards and best practices.

Key Responsibilities:

Targeted HIV Outreach and Engagement:

• Conduct targeted HIV outreach activities in priority communities consistent with Ryan White Part B guidelines and local epidemiologic data.
• Identify and engage populations in need, including individuals at increased risk for HIV or those who are out of care or newly diagnosed.

HIV Prevention and Education:

• Deliver community-based HIV prevention education, including information on condom use, Pre-Exposure Prophylaxis (PrEP), Post-Exposure Prophylaxis (PEP), and routine testing.
• Distribute prevention materials and educational resources in accordance with program policies.
• Support stigma-reduction efforts through culturally responsive education and outreach activities.

Linkage to Care and Support Services:

• Facilitate referrals and warm handoffs to medical and supportive services as appropriate.
• Coordinate with internal staff and external partners to ensure timely linkage to services.
• Assist clients with navigating healthcare and support systems, understanding available resources and eligibility requirements.

Data Collection and Reporting:

• Accurately document activities, encounters, and referrals in required systems and reports.
• Collect required outreach and performance data in compliance with reporting requirements.
• Participate in quality improvement activities related to outreach effectiveness and client engagement.

Community Collaboration and Representation:

• Represent Afya at community events, health fairs, and coalition meetings as assigned.
• Maintain positive working relationships with community partners, healthcare providers, and local organizations.
• Support agency-wide prevention and public health initiatives as needed.

Qualifications:

• High school diploma or GED required;
• Knowledge of HIV prevention, testing, and care concepts, or willingness to obtain training.
• Strong interpersonal, communication, and engagement skills.
• Ability to work flexible hours, including evenings or weekends, as needed for outreach activities.
• Valid driver’s license and reliable transportation.

Working Conditions:

• This position involves frequent community-based work in non-traditional settings.
• May require standing for extended periods, walking, and lifting outreach materials.
• Travel within the service area is required.
• Work schedule may vary based on outreach events and community needs.

Job Types: Part-time, Contract

Pay: $20,000.00 per year
