About the Role

AfriThrive is seeking compassionate, motivated, and community-connected Community Health Worker (CHW) to support our ThriveRx Program in Baltimore County, particularly Essex and surrounding communities.

CHWs will serve as trusted connections between program participants, healthcare providers, and community resources. • Maintain regular contact with assigned participants through phone calls, text messages, and in-person check-ins to confirm receipt of food deliveries, address barriers to participation, follow up on missed services, and support ongoing      engagement and retention.
  • Provide culturally responsive health and nutrition education, explain how nutritious foods support chronic disease management, identify additional participant needs, and connect individuals and families with appropriate healthcare and community resources.
  • Serve as a trusted liaison between AfriThrive, participants, and community partners; follow up with prospective participants who do not enroll immediately, coordinate referrals, resolve service concerns, and support outreach events and related logistics.
  • Complete accurate and timely enrollment forms, participant records, follow-up documentation, and program reports; track referrals and services provided while maintaining confidentiality and complying with applicable privacy and program
