Reporting Relationships: Case Manager report to the Director of Social Services

What You Will Do

Under the general supervision of the Director of Social Services, Case Managers are expected to perform the following principal duties and responsibilities.

  • Conduct initial intake including but not limited to social service assessments, psychosocial, independent living plans, and referrals of incoming clients who are referred from the shelter system
  • Conduct updates to psychosocial assessments and other evaluations as a basis for developing appropriate referrals, changes in treatment planning, and other interventions.
  • Identify benefits and entitlement and connect clients to mental health, medical treatment & substance use treatment facilities where applicable
  • Obtain identification, public benefits and other applicable income sources for clients
  • Meet biweekly with clients to complete Individual Living Plan to assess ongoing needs, encourage harm reduction, and make referrals for services
  • Document all efforts in providing services, interactions, follow-up regarding resident involvement
  • Manage and track employment placements of shelter residents
  • Maintain up-to-date confidential client case records for assigned caseload
  • Serve as a liaison to appropriate support service agencies and programs e.g., Health facilities; Income Maintenance Center, Board of Education, etc.)
  • Complete all required trainings in a timely manner
  • Teach residents how to advocate for themselves when seeking employment in the community
  • Assist clients in attaining their employment-related goals by identifying and referring clients to appropriate services
  • Assist the Program Director and Director of Social Services to ensure performance objectives, including placements goals for the number of residents rehoused, are met
  • Work with all DHS, HRA, government bodies and other resources to advocate for and ensure client housing needs are being addressed in a timely manner
  • Conduct workshops for residents on the housing search, application, and interview process on an as needed basis
  • Participate all mandated meetings including social services, team meetings and agency-wide meetings
  • Complete all required agency and funder trainings in a timely manner
  • Perform other related duties as assigned

Qualifications

About you:

Degree Requirement: Bachelor’s degree relevant to human services and at least one (1) year of proven experience in Human Service related field or;

  • An Associate Degree with at least two (2) – three (3) years of proven experience in a Human Service related field.
  • Bi-lingual preferred;
  • Familiarity with CARES, preferred.

  • Must possess sensitivity, tact, and resourcefulness.
  • An eagerness to assist clients in a nonjudgmental, empathetic and compassionate manner.
  • Must demonstrate responsiveness to cultural differences and a commitment to the value of cultural competency.
  • Ability to develop and maintain appropriate boundaries.
  • Excellent organizational, time management and writing skills.
  • Experience working with DHS and other service providers, Preferred.
  • Experience providing case management services to families with children referred from DHS.

Compensation And Benefits

Base salary range and benefits information for this position are being included in accordance with requirements of New York State and City pay transparency laws. Please note that salaries may vary for different individuals in the same role based on several factors, including but not limited to location of the Role, Individuals Competencies, Education/professional Certifications, Qualifications/experience, Performance in the role.

Compensation

The base salary compensation range being offered for this role is $61,807..
