Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by checking vital signs and observing for any changes in condition or behavior
- Administer medications accurately following care plans and HIPAA privacy regulations
- Prepare nutritious meals and assist with meal planning to support dietary needs
- Use specialized equipment such as Hoyer lifts or transfer belts to ensure safe patient transfers and mobility assistance
- Maintain detailed documentation of care provided, patient progress, and any incidents or concerns
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe living environment
- Support clients with cognitive impairments such as dementia or Alzheimer’s through memory care techniques and patient engagement
- Collaborate with healthcare professionals to develop and follow personalized care plans
- Provide emotional support, companionship, and social interaction to enhance quality of life
Experience
- Prior experience working in home health, assisted living, nursing homes, or group homes is highly preferred
- Demonstrated knowledge of patient observation, medication administration, vital signs monitoring, and care planning
- Experience caring for individuals with developmental disabilities, autism spectrum disorder (ASD), dementia, or hospice needs is advantageous
- Familiarity with HIPAA regulations and maintaining patient confidentiality is essential
- Hands-on experience with patient transfer equipment such as Hoyer lifts or wheelchairs is desirable
- Strong background in providing long-term care or social work services within home or community settings
- Ability to handle heavy lifting safely and perform physically demanding tasks as needed
