Job Summary
We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) to join our healthcare team. Duties
- Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating physiological responses.
- Administer medications and injections accurately while adhering to prescribed protocols and maintaining strict compliance with HIPAA regulations.
- Assist with invasive procedures such as IV infusion, catheterization, and sterile dressing changes using aseptic techniques.
- Document patient care activities thoroughly within electronic health record (EHR) systems like Epic or Cerner to ensure precise medical records management.
- Support discharge planning and coordinate post-acute care services for patients transitioning from hospital to home or long-term care facilities.
- Provide specialized care for individuals with dementia, Alzheimer's disease, or developmental disabilities, including memory care and behavioral health support.
- Manage airway maintenance, ventilator support, and emergency response procedures in high-acuity environments such as ICU or Level I Trauma Centers.
- Collaborate with physicians and other healthcare professionals during diagnostic evaluations like sonography or laboratory specimen collection to optimize patient outcomes.
Experience
- Proven clinical experience in diverse settings such as hospital medicine, emergency room, ICU, or medical-surgical units.
- Demonstrated expertise in critical care environments including Level I Trauma Centers or Level II Trauma Centers with acute pain management skills.
- Experience working with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks for accurate documentation and coding (ICD-9/10; CPT).
- Background in specialized fields including geriatrics, hospice care, infectious disease management, or pediatric nursing is highly desirable.
- Prior work with individuals with disabilities or in home & community care settings to deliver personalized patient-centered services.
- Knowledge of physiology, anatomy, pharmacology related to medication administration and medical terminology is essential.
