Registered Nurse (RN) Case Manager – New Haven County

Affirmed Home Care—Connecticut's leading private-pay home health agency—is seeking an experienced and compassionate Per Diem RN Case Manager to support clients across New Haven County, CT. This field-based role is ideal for a self-directed nurse who values autonomy, meaningful client relationships, and delivering high-quality, personalized care in the home setting.

Key Responsibilities

• Provide and uphold exceptional standards of in-home client care

• Create, implement, and monitor individualized care plans

• Secure and maintain physician orders in accordance with state regulations

• Observe, document, and communicate changes in client condition

• Supervise and support home care staff, promoting strong clinical practices

• Help clients maintain independence, comfort, and safety at home

Qualifications

• At least two years of recent RN experience in an acute care setting; home care experience a plus

• Clinical proficiency in:

  • Ventilator and tracheostomy care

  • G-tube/PEG tube care

  • IV infusions and wound care

• Active Connecticut RN license and current BLS certification

• Recent physical (within 1 year) and PPD/QuantiFERON (within 1 year or chest X-ray within 5 years)

• Driver’s license preferred

• Authorization to work in the United States
