Summary

The Registered Nurse On-Call is responsible for providing after-hours hospice care, ensuring patients receive timely and compassionate support. This role involves planning, organizing, directing, and delivering nursing services in collaboration with the interdisciplinary team. The RN On-Call will develop individualized care plans, address urgent patient needs, and assist families in maintaining comfort and coping during after-hours emergencies.

Hours / Schedule

Full-time, 7 days on / 7 days off (on-call role), Monday through Monday

• Monday–Friday: On call from 5:00 PM to 8:00 AM. Respond to calls and complete visits as needed.
• Weekend Coverage (Saturday–Monday): On call continuously through Monday at 8:00 AM, including answering calls and completing visits as needed. Triage all after-hours needs during this period.
  
  

Position Qualifications

• Associate degree in nursing from an accredited college or university (or international equivalent) preferred.
• Minimum of two (2) years of hospice nursing experience required.
• Current RN license in the assigned state.
• Current CPR certification is required.
• Valid driver’s license with a reliable, insured vehicle that meets organizational requirements.
  
  

Responsibilities

• Ability to work on call night and weekends as needed.
• Work closely with the sales, clinical, and leadership team.
• Case manages and provides clinical direction to the interdisciplinary team, physician, and family. 
• Implement/develop/document the patient plan of care. 
• Provide care utilizing infection control measures that protect both the staff and the patient (OSHA). 
• Assure continuity of quality patient care delivered with appropriate documentation. 
• Monitor assigned cases to ensure compliance with requirements of third-party payer. 
• Demonstrate commitment, professional growth, and competency. 
• Promote agency philosophy and administrative policies. 
• Perform on-call responsibilities and provide on-call service to patients/families as assigned. 
• Supervise and provide directions to the home health aide and the LPN. 
• Perform other duties as assigned by management to support department goals and initiatives.
