Key Responsibilities

  • Supervise, mentor, and support clinical staff across home care, assisted living, and skilled nursing facility settings.

  • Ensure timely assessments, plan of care development, and interdisciplinary team (IDT) meetings for hospice patients.

  • Monitor and ensure compliance with state and federal regulations (including Medicare hospice benefit requirements, documentation, licensure) as well as internal policies and accreditation standards.

  • Oversee staff scheduling, oncall rotation, visit coverage, and staffing models to maintain patient care continuity.

  • Participate in budgeting and resource planning for clinical operations as needed; monitor supply usage and equipment needs.

  • Promote the organization’s mission and values: compassionate, dignified care for patients and families; teamwork; and professional development.

  • Prior supervisory or leadership experience in a clinical setting preferred.

  • Familiarity with hospice regulatory requirements, clinical documentation, IDT processes, and interdisciplinary care.

  • Strong leadership, coaching and mentoring skills — able to develop and support a highperforming clinical team.

  • Excellent interpersonal and communication abilities — able to work with patients, families, clinical staff, and leadership.

  • Proven ability to manage complex clinical operations, prioritize multiple tasks, handle change and work in a fastpaced environment.

  • Strong clinical judgment, problemsolving skills, and comfort with endoflife care and hospice philosophy.

  • Excellent organizational skills, attention to detail, and competency in electronic medical records and documentation systems.

  • Commitment to maintaining clinical excellence, quality improvement, patientcentered care, and regulatory compliance.
