This role requires strong communication skills to coordinate services between the palliative team, patients, and families via telephone. The Clinical Resource Specialist provides skilled professional nursing care under the supervision of a mid-level provider while ensuring compliance with applicable laws, regulations, and company policies. The specialist is also responsible for maintaining thorough documentation of patient services, coordinating services and equipment with contracted vendors, and following up on pending patients, referrals, and those not taken under care (NTUC). Additionally, this role involves utilizing the company’s computer systems for processing admissions and medical records, managing expenses responsibly, and traveling as needed. Hours / Schedule:  Part-time Minimum Qualifications: Graduate of an accredited practical nursing or vocational nursing program. High school diploma or equivalent. Minimum of one (1) year of general nursing experience (community health, hospice, or medical/surgical experience preferred). Customer service experience required (hospice or medical office experience preferred). Current LPN/LVN licensure in the state of practice. Valid driver’s license Key Responsibilities: Ensure quality and safe delivery of palliative care services to patients, families, and caregivers. Implement nursing care plans under the direction of the interdisciplinary team and supervising mid-level provider. Facilitate all palliative admissions, ensuring a seamless transition for patients. Educate patients and families regarding diagnoses and self-care plans. Provide skilled nursing care using all elements of the nursing process as directed by the care plan. Document patient services accurately and promptly in compliance with care plans. Perform daily data entry and maintain electronic records. Process weekly payroll for Palliative Care Mid-Levels, verifying visit completion. Monitor and resolve billing errors for held claims. Prepare referral statistics reports for daily stand-up meetings. Maintain census and referral boards to track patient activity. Assist the Palliative Care Intake Coordinator with scheduling mid-level visits and admissions. Maintain strong internal and external communication via phone and fax. Assist patients, families, caregivers, and team members in ensuring continuity of care. Complete and maintain patient records at residences and submit accurate clinical notes. Maintain clinical records, reports, and statistics in compliance with legal and regulatory requirements. Provide pain and symptom management and evaluate patient response to treatment. Facilitate communication and referrals to additional interdisciplinary team members (e.g., OT, PT, MSW, Dietitian). Assist with IDG process meetings and agendas. Coordinate initial services and equipment with contracted vendors. Order and manage medications as needed. Participate in marketing efforts, seeking new referrals and building relationships with potential sources. Support family and caregiver needs, ensuring emotional and clinical support. Monitor patient conditions and report changes to supervisory personnel. Affinity Hospice is an equal opportunity employer.
