Qualifications & Skills: Licensed Physical Therapist with relevant clinical experience, strong interpersonal and communication skills, and ability to work independently. Knowledge of SNF protocols and patient-centered care is preferred.
Key Duties: Conduct patient assessments, develop and adjust treatment plans, monitor progress, and ensure compliance with healthcare regulations. Perform therapy sessions during assigned shifts, documenting outcomes and communicating with team members.
