- Provide one-on-one physical therapy to older adults in private residences and senior living communities
- Perform evaluations, develop personalized care plans, and deliver evidence-based treatments
- Support patient wellness, mobility, independence, and aging in place through therapy sessions
- Document within EMR systems adhering to regulatory standards
- Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists
- Educate patients, caregivers, and staff on safety and functional techniques
- Utilize telehealth and virtual care to enhance access and continuity of care
- Valid Physical Therapy license or eligibility in practice state(s)
- Degree from an accredited Physical Therapy program
- Proficiency in basic computer use and EMR systems
- Current CPR certification
- Ability to exert medium physical effort, including lifting, repositioning, and assisting patients
- Perform CPR and assist with patient transfers
- Extended standing, walking, climbing stairs, and driving to patient locations
- Wearing PPE as required
