Role: Hospice Registered Nurse responsible for pain management and symptom control for hospice patients, following the plan of care and collaborating with an interdisciplinary team, attending physician, and Hospice Medical Director.

- Provide timely hospice services, assessing patient and family needs during home visits and documenting interventions and outcomes.
- Coordinate care with team members, attending physicians, and other providers, ensuring continuity across care settings.
- Participate in developing, reviewing, and updating the patient/family plan of care, and communicate progress and needs.
- Supervise hospice aides and licensed practical nurses, ensuring compliance with care plans and documentation standards.
- Facilitate care in long-term care facilities, document joint care, and attend facility care meetings as needed.
- Contribute to ongoing patient monitoring, certification, and recertification processes.
- Engage in on-call rotations for after-hours, weekend, and holiday care delivery.
- Active RN license in the applicable state.
- Valid CPR certification.
- Preferred: At least 1 year of hospice experience.
