Affinity Healthcare Solutions is looking for skilled, compassionate Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide shift care for a Peg-tube, trach and vent dependent patient in the 19090 zip code area of Willow Grove. This is a patient who needs round the clock care with consistent scheduling, offering an opportunity for nurses to provide one-on-one, high-quality skilled nursing care in a home setting.

Responsibilities
- Deliver skilled nursing care to trach/vent-dependent patients in their home
- Conduct thorough assessments and maintain accurate medical records
- Monitor vital signs and patient conditions, and report changes promptly
- Administer medications and other prescribed treatments
- Oversee fluid intake/output and maintain comprehensive patient charts
- Collaborate with the family andinterdisciplinary healthcare team to ensure optimal care
- Provide education, emotional support, and guidance to patients and caregivers
- Adhere to all clinical and regulatory standards to ensure patient safety and quality of care
- Active RN or LPN License in Pennsylvania (LPNs may apply to other postings)
- Trach and vent experience is required
- At least 1 year of clinical or home health experience preferred
- Strong critical thinking and assessment skills
- Familiarity with infection control protocols, nursing best practices, and safety standards
- Excellent communication and interpersonal skills
- Proficiency in documentation and use of electronic charting systems (Axxess and HHAeXchange experience a plus)
- Reliable, compassionate, and able to work independently in a home care setting
