Skilled Nursing - Private Duty

Affinity Healthcare Solutions is looking for skilled, compassionate Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide shift care for a Peg-tube, trach and vent dependent patient in the 19090 zip code area of Willow Grove. This is a patient who needs round the clock care with consistent scheduling, offering an opportunity for nurses to provide one-on-one, high-quality skilled nursing care in a home setting.

Immediate start is available for qualified candidates who are passionate about patient care and reliability.

We use Axxess and hhaexchange.

Responsibilities

• Deliver skilled nursing care to trach/vent-dependent patients in their home
• Conduct thorough assessments and maintain accurate medical records
• Monitor vital signs and patient conditions, and report changes promptly
• Administer medications and other prescribed treatments
• Oversee fluid intake/output and maintain comprehensive patient charts
• Collaborate with the family andinterdisciplinary healthcare team to ensure optimal care
• Provide education, emotional support, and guidance to patients and caregivers
• Adhere to all clinical and regulatory standards to ensure patient safety and quality of care
  
  

Skills

• Active RN or LPN License in Pennsylvania (LPNs may apply to other postings)
• Trach and vent experience is required
• At least 1 year of clinical or home health experience preferred
• Strong critical thinking and assessment skills
• Familiarity with infection control protocols, nursing best practices, and safety standards
• Excellent communication and interpersonal skills
• Proficiency in documentation and use of electronic charting systems (Axxess and HHAeXchange experience a plus)
• Reliable, compassionate, and able to work independently in a home care setting
