Director Of Field Operations- Job Description

Organization: Continuum Hospice and Palliative Care / Shalom Hospice

Reports To: Vice President of Operations

Classification: Full-Time, Exempt

Location: Washington State, travel required throughout Snohomish, Pierce, Kitsap, King, and Whatcom Counties as needed

Position Summary

The Director of Field Operations provides operational and clinical support, as well as leadership oversight to field-based hospice services across multiple counties and interdisciplinary teams. This role partners closely with Clinical Managers, the Clinical Director, and executive leadership to ensure high-quality, compliant, efficient, and compassionate hospice care delivery.

The Director of Field Operations is responsible for supervising Clinical Managers and Admission Nurses, supporting staff development, overseeing operational performance initiatives, participating in Quality Assurance and Performance Improvement (QAPI) activities, and ensuring consistent standards of care across the organization. This position serves as a key leader in advancing organizational goals, staff engagement, regulatory compliance, and patient satisfaction.

Essential Duties And Responsibilities

Leadership & Operations

  • Provide direct supervision, mentorship, and operational oversight to Clinical Managers overseeing multiple clinical teams.
  • Provide direct supervision and scheduling of Admission Nurses.
  • Support Clinical Managers in daily management of clinical teams, staffing coordination, workflow planning, and problem-solving.
  • Assist Clinical Managers in developing leadership skills, including coaching, conflict resolution, accountability, and team performance management.
  • Collaborate with leadership to ensure continuity and consistency of hospice operations across all service areas.
  • Participate in strategic planning and operational initiatives to improve quality, productivity, and patient outcomes.
  • Support implementation and oversight of organizational programs, including Vitalis programs and related field initiatives.
  • Participate in on-call leadership support as assigned.


Staff Development & Human Resources

  • Conduct annual performance evaluations for Clinical Managers and Admission Nurses.
  • Provide ongoing coaching, counseling, corrective action, and performance improvement support as needed.
  • Assist Clinical Managers in developing skills and techniques for evaluating clinician performance.
  • Participate in recruitment, interviewing, hiring, onboarding, and retention efforts for supervised staff.
  • Participate in in-person employee orientation and onboarding activities.
  • Ensure compliance with organizational policies, standards of conduct, and regulatory requirements.
  • Support employee engagement, professional growth, and a positive team culture.


Clinical Oversight & Quality Improvement

  • Collaborate with the Clinical Director/VP and leadership team in the evaluation of organizational performance through the QAPI program.
  • Assist with productivity monitoring, quarterly reviews, annual program evaluations, and performance improvement initiatives.
  • Assure the quality and safe delivery of hospice services throughout the organization.
  • Monitor operational and clinical trends and assist with corrective action planning when necessary.
  • Support regulatory readiness and compliance with federal, state, and accreditation standards.
  • Participate in audits, chart reviews, and quality monitoring activities as assigned.


Admission Team Oversight

  • Supervise Admission Nurses and support excellence in admission processes and patient experience.
  • Coordinate and lead monthly Admission Nurse in-services and educational meetings.
  • Monitor admission workflows, timeliness, documentation standards, and productivity expectations.
  • Provide guidance and support related to hospice eligibility, customer service, communication, and clinical standards.


Communication & Collaboration

  • Maintain effective communication with physicians, interdisciplinary teams, community partners, and leadership.
  • Foster collaboration between operational and clinical departments to support organizational goals.
  • Participate in leadership meetings, committees, and organizational initiatives.


Qualifications

  • Current unrestricted Registered Nurse (RN) license in the State of Washington required.
  • Minimum of 3–5 years of hospice leadership or healthcare management experience preferred.
  • Experience supervising clinical staff and managing multidisciplinary teams required.
  • Strong knowledge of hospice regulations, Conditions of Participation, and quality standards.
  • Demonstrated leadership, organizational, communication, and problem-solving skills.
  • Experience with QAPI and performance improvement initiatives preferred.
  • Valid driver’s license, reliable transportation, and ability to travel throughout assigned counties required.


Physical Requirements

  • Ability to travel regularly between office and field locations.
  • Ability to sit, stand, walk, bend, lift, and perform duties associated with office and field-based healthcare operations.
  • Ability to utilize computers, phones, and electronic medical record systems effectively.


Work Environment

This position functions in both office and field environments, including patient homes, assisted living facilities, skilled nursing facilities, and organizational offices. Travel throughout Snohomish, Pierce, Kitsap, King, and Whatcom Counties may be required based on operational needs.
