Position Qualification Requirements

Experience
- Five (5) years' experience in ambulatory care, hospital and /or community health nursing.
- Experience in ambulatory care, hospital or community health nursing.
- Good organizational, analytical, and problem-solving skills.
- Excellent verbal and written communication skills.
- Able to work with people with diverse ethnic backgrounds.
- Strong computer skills including the use of word process, spreadsheet and e-mail software.
- Able to work on multiple tasks simultaneously.
License(s)/Certification(s) Required: Licensed RN in Missouri and CPR certified.
Essential Functions
FUNCTIONS OF THE JOB
- Work with health coaches' compliance to established policy and procedures with daily work schedules.
- Recruit, patients with Chronic Disease conditions and work with qualified health coaches and BCCCP Follow-up Assistant.
- Coordinate and implement training, educational classes and in-services for staff in chronic disease management, patients and JACHO standards.
- Assist in the development and implementation of policies and procedures for Chronic Disease department and other areas as required.
- Coordinate and oversee special programs as assigned.
- Prepare and maintain needed records and reports.
- Prepare and revise educational materials for clients with chronic diseases.
- Complete outreach/in reach and follow up activities provided through BCCCP.
- Participate as a member of the Health Disparities Collaborative teams dealing with chronic diseases.
- Work in coordination with health center nurse coordinators and supervisors to monitor and maintain adherence to JCAHO and other accrediting body standards.
- Represent Affinia Healthcare in the community and make presentations as requested.
- Monitor the goal achievement of Healthy Heart Program and other grants and contracts involving coaching of clients with chronic diseases.
- Participate in the planning and evaluation of new and existing programs.
- Perform other duties as assigned.
