Role Description
The Patient Management Nurse is a full-time on-site position located in Clermont, FL. In this role, you will be responsible for managing patient care by conducting assessments, creating and monitoring care plans, coordinating with physicians and healthcare teams, and ensuring continuity of care for patients. Your daily tasks will include communicating with patients and caregivers, performing follow-ups, and addressing patients' medical concerns to improve overall health outcomes. You will play a vital role in guiding patients through the healthcare system with empathy and professionalism.
Qualifications
- Strong clinical nursing skills, including patient assessments, care planning, and treatment monitoring
- Experience in patient communication, education, and providing empathetic, patient-centered care
- Proficiency in medical record documentation and case management protocols
- Effective collaboration skills with healthcare teams, including physicians, specialists, and support staff
- Proficiency with electronic medical records (EMR) systems and basic computer skills
- Active RN/LPN license in the state of Florida
- Ability to work on-site in Clermont, FL
- BLS/ACLS certification preferred
- Experience in chronic disease management is a plus
