Start 2026 by helping adults with mental health challenges build skills, independence, and community. Advocate Support Services, Inc. delivers mobile, community‑based PRP services across Anne Arundel, Howard, Prince George’s, Baltimore City, and Baltimore County—minimum five (5) meaningful contacts per month, with telehealth when appropriate.

What you’ll do
- Provide rehabilitative visits in the community (home and neighborhood settings) focused on ADLs, coping skills, social integration, budgeting, transportation, and health navigation; meet or exceed the 5‑visit monthly standard.
- Coordinate care with therapists, prescribers, and community partners; document collateral contacts and care plans.
- Use person‑centered goals (IRP) and update progress notes promptly and accurately in the EHR.
- Collaborate with your Rehabilitation Specialist and supervisor to ensure quality, compliance, and timely documentation.
You bring
- Experience supporting adults with SMI/serious behavioral health needs (community work preferred).
- Strong communication, organization, and follow‑through in mobile field work.
- Comfort with documentation, EHRs, and weekly visit planning; detail‑oriented note quality.
- Valid driver’s license/reliable transportation; ability to work in assigned territories (AA/HoCo/PG/Baltimore City/Baltimore County).
Nice‑to‑have
- CPRP or progress toward certification; bilingual skills; prior PRP experience.
Schedule & location
Mobile/field‑based with flexible scheduling; some Telehealth; weekly supervision and team collaboration.
