Skills/Qualifications:
- Bachelor’s degree in social work, psychology, human services, or related field.
- Bilingual English/Spanish or English/Creole
- Obtain Child and Adolescent Needs and Strengths standard CANS comprehensive 3.0 Certification within 30 days of hire.
- Minimum of two years working with children, youth, and families in community and home-based settings.
- Knowledge of Miami-Dade community and resources preferred.
- Excellent written and oral communication skills.
- Able to work with teams

- Strong commitment to working with families from their strengths and protective factors.
- Ability to work independently.
- Strong computer skills (Microsoft, word, Excel, Access, PowerPoint, and Outlook).

Responsibilities:
- Provide intensive home-based services in partnership with family to create a Success Plan, make referrals, facilitate access to services and monitor to ensure that all components of Success Plan may be accomplished
- Receives cases assigned by Program Manager and maintains a caseload of 15-25 families, schedules home / community visits with families for intake and assessments.
- Assist with collection of pre and post-data on families.
- Works effectively with team members and provides assistance to the Child and Family Specialist.
- Convenes child/family specific team, provides facilitation of Success Plan of services.
- Enter Success Plan in database and track service access, barriers, and gaps.
- Work with families in scheduling service appointments and monitor service utilization.
- Attends staff meetings, conferences, and workshops regularly to upgrade professional skills.
- Maintain filing system for all cases and keep records of all contacts with families and providers
- Work with the family to identify long-term, ongoing community helpers from among their service providers and natural helpers
- Through ongoing relationship building with the family, help facilitate nurturing, contingent, parent-child interactions.
- Participate in group supervision/case conferencing and schedule individual supervision meetings.
- Miscellaneous Duties distributed by the Program Manager and Program Director.
