Company Overview:
The Advocate Program, Inc. is a private, not-for-profit, 501 c (3) justice agency that offers probation, diversion, community service, DUI and traffic programs, domestic violence programs, substance abuse and mental health evaluations and treatment to court-ordered individuals. It has expanded to work with juveniles and families in the juvenile justice field, families impacted by family conflict and violence, and homeless veterans and their families.
SUMMARY:
The Care Coordinator manages direct service staff that manage a client caseload to manage service integration, treatment planning coordination, transitional planning, and linkages.
Education and/or Experience:
Bachelor’s degree in nursing, Social Work, Healthcare Administration, or a related field. 1-2 years of experience in care coordination, case management, or related field.
Qualification Requirements:
- Strong understanding of case management/care coordination principles and recovery-oriented system of care principles.
- Strong organizational skills with the ability to manage multiple client cases simultaneously.
- Excellent communication and interpersonal skills to work with diverse client populations and healthcare teams.
- Proficiency in electronic health record systems and basic computer skills.
- Knowledge of community resources, insurance processes, and healthcare regulations.
- Empathy, patience, and a passion for supporting clients through their care journey.
Responsibilities:
- Conduct initial assessments of client needs, goals, and barriers to accessing care.
- Develop individualized care plans in collaboration with service providers, clients, natural supports and families.
- Update care plans regularly based on changes in client condition or treatment needs.
- Coordinate appointments, referrals, and follow-up care to ensure clients receive timely services.
- Communicate with providers, specialists, and other healthcare services to facilitate smooth transitions in care.
- Monitor and track client progress, following up to address any issues with appointments, treatments, or access to services.
- Serve as a central point of contact for patients and their families, offering guidance and support in navigating the continuum of care.
- Advocate for clients’ needs, helping them understand their treatment options, care plans, and resources available.
- Maintain accurate records of client interactions, care plans, and coordination activities
- Ensure all documentation meets regulatory and compliance standards.
- Miscellaneous Duties as assigned.
