Coordination of "Important Message from Medicare" (IM) correspondence.
- Obtains signatures from Medicare Beneficiaries or their emergency contact within 48 hours prior to discharge as directed by the Care Manager or Social Work Staff.
- Mails out appropriate letters to emergency contacts when the Medicare Beneficiary is unable to take receipt of the acknowledgement or when directed by the Care Manager of Social Work Staff.
- Monitors all department voicemail lines and follows up appropriately.
- Delivers the IM letter/Code 44 letter to the Medicare Beneficiary or their emergency contact within 48 hours prior to discharge as directed by the Care Manager or Social Work Staff.
- Documents all activities related to the IM/Code 44 in Care Connection.
- Provides clerical support to the Care Managers and Social Work Staff during the appeals process for a Medicare Beneficiary.
- Provides required documents to vendors in a timely manner, faxing appropriate information from the patient's medical record when necessary.
- Assists with facility bed availability and updates the Care Manager or Social Worker of status of facility as needed until patient discharge when necessary.
- Identifies contracted vendors for discharge relating to home health care, nursing home placements, durable medical equipment, LTACHs, Hospice etc.
- Monitors all department voicemail lines including the social work voicemail and follows up appropriately.
- Updates hospital systems related to discharge.
- Provides reports as needed as directed by the CM Leadership.
- Order supplies for the department.
- Take meeting minutes for departmental as well as ad hoc meetings when requested. Education/Experience Required:
- H.S. diploma or equivalent
- 2-3 years clerical experience in a health care setting
- Ability to use Microsoft Office
- Ability to handle difficult calls and maintain professional conduct
- Good communication skills
