- Assures the accurate and timely completion of IRF-PAI.
- 1)Keeps track of the need for admission and discharge assessments for each patient, informing the team in a timely manner of the need for data.
- 2)Enters IRF-PAI data from various documented assessments, as well as interviews with the patient, family, physician, or clinicians involved in the care of the patient.
- 3)Completes portions of the IRF-PAI assessment and documents accordingly.
- 4)Verifies accuracy of all data recorded, reviewing data from others for possible conflicting information.
- 5)Enters IRF-PAI data accurately into the computer.
- 6)Submits data in a timely manner.
- Coordinates admissions to the Comprehensive Inpatient Rehabilitation Unit with the physiatrist.
- 4)Communicates with referral sources regarding status of patient referral.
- 5)Communicates with patients and their families regarding eligibility for admission.
- 6)Provides recommendations to referral sources for patients determined to be inappropriate for admission.
- 7)Monitors bed assignment with the charge nurse in order to coordinate the admission to the unit and ensure patient needs are addressed.
- 8)Analyzes data on referrals for clinical and financial trends. Provides input to QI and Program Planning activities based on analyzed data.
- 1)Receives and tracks referral information, monitoring efficient accomplishment of referral process and overseeing distribution of referrals to physician. For referrals from outside Advocate Lutheran General Hospital, obtains and reviews all necessary clinical and financial information, performs onsite clinical assessment if necessary to determine appropriate level of care, discusses assessment information and patient eligibility with physiatrist.
- 2)Provides patient financial information to the business office and coordinates with the business office regarding patient eligibility for comprehensive inpatient rehabilitation. Completes pre-certification / certification process if required.
- 3)Ensures that medical stability issues are resolved prior to transfer.
- Assures the accurate and timely completion of UDS-FIM data.
- 1)Ensures the collection of admission, interim, and discharge FIM ratings on all patients in a timely manner from the appropriate clinicians.
- 2)Ensures that the UDS-FIM data is received and routed to the appropriate support personnel for entry into the computer.
- 3)Assists in interpretation of the UDS-FIM outcome data reports, and in developing a plan of action to optimize patient outcomes.
- 4)Ensures the collection of Follow up data.
- Participates in the development and modification of the patient plan of care.
- 1)Participates in using the IRF-PAI and UDS-FIM data to develop and modify individual plans of care for the patient.
- 2)Attends patient rounds.
- 3)Assures that all patient care plans are coordinated with the IRF-PAI.
- 4)Acts as a resource to staff in the development and content of the plan of care.
- Facilitates collaboration and communication among members of the patient’s health care team.
- 1)Verifies the congruity of the patient's status between the FIM-UDS, IRF-PAI and the medical record.
- 2)Actively participates in QA activities and program development.
- 3)Provides staff education leading to increased staff involvement.
- 4)Works with finance, medical records, and IS.
- 5)Brings all conflicts in data to the attention of the team, and facilitates a resolution.
- Assists in leadership of facility staff in relation to IRF-PAI and UDS-FIM requirements.
- 1)Keeps abreast of the latest regulatory standards to assure facility compliance with all requirements.
- 2)Serves as a resource / mentor to associates on the rehabilitation team regarding the IRF-PAI and the UDS-FIM program.
- 3)Maintains communication with the Director of Rehabilitation, Clinical Manager, and unit Medical Director regarding issues related to the IRF-PAI and UDS-FIM processes.
Licensure:
- Nurse, Registered (RN)
Education/Experience Required:
- * Eff. 6/1/2012: BSN preferred, ADN with agreement to enroll in BSN completion program in two years to be completed in five years. Graduate of an approved school of nursing Bachelor's Degree 2 to 3 years experience in rehabilitation nursing.
Knowledge, Skills & Abilities Required:
- Strong interpersonal skills and communication. Highly detail focused and organized Good problem solving skills. Knowledgeable in use of personal computers
- Licensed in the state of Illinois as a Registered Nurse
Physicial Requirements and Working Conditions:
- Ability to work various days during the week and on weekends
- If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
