What are the responsibilities and job description for the Hospice Chaplain MKE South Waukesha Jefferson position at Advocate Aurora?
Major Responsibilities:
- Provides initial assessment of existing spiritual resources and preferences of patients.
- Provides spiritual care and emotional support to patients and families. In conformance with the patient's faith and expressed desire, pastoral care services of counseling, sacraments, and support will be available.
- Responsible for development of a spiritual plan of care reflecting problems, interventions, and outcome goals.
- Implements the plan of care in conjunction with the hospice interdisciplinary team and according to hospice specific protocols and measures.
- Serves as liaison for the hospice patient and their local faith community.
- Facilitates conversations specific to end-of-life planning including goals of care, funeral arrangements, and legacy work.
- Collaborates with hospice interdisciplinary team with referrals of patients who are in crisis situations.
- Participates in the QA/CQI process for Advocate Hospice.
- Responsible for achievement of agency, department and personal goals and objectives.
Licensure, Registration and/or Certification Required:
- A valid driver’s license issued by the Division of Motor Vehicles.
Education/Experience Required:
- Master's Degree in Theology, or Master's Degree in Divinity, or Master's Degree in Religious Studies.
- At least Two (2) units of CPE, accredited by ACPE; NACC units may be considered. 5 units preferred.
- In good standing with continued endorsement by the appropriate authority of his/her church or religious institute
Knowledge, Skills & Abilities Required:
- A strong background in functional theology and the ability to apply this in a pastoral manner to individuals and families.
- Excellent communication skills with knowledge of counseling techniques.
- Ability to identify client’s needs with consideration to cultural, ethnic, religious, economic, and other diverse backgrounds.
- Experience and compassion in ministering to people who are dying and their grieving families.
