Major Responsibilities:
Clinic Operations & Patient Engagement
- Assist with patient intake, scheduling, and follow-up to ensure continuity of care.
- Support and promote clinic programs, processes, and services, reducing barriers to care.
- Oversee clinic processes such as eligibility, missed appointments, late arrivals, and patient assistance programs to improve access to care, patient experience, and health outcomes.
- Schedule specialty care appointments and connect patients to insurance application support.
- Advocate for patient needs and link to resources.
- Maintain accurate documentation in EHR and other systems.
- Track data and assist with grant reporting.
Care Coordination & Social Support
- Conduct outreach for preventive services (e.g., screenings, vaccinations, advanced directives). Reduce barriers to ensure completion of appointments as well as improved ambulatory quality composite metrics.
- Assist with medication and vaccination applications for patient assistance programs.
- Facilitate health promotion activities aligned with patient needs and funding requirements.
- Perform SDOH screenings (housing, food, transportation, employment) and connect patients to appropriate resources.
- Provide culturally responsive peer support and education, including home visits when needed.
Team and Community Collaboration
- Collaborate with staff to reduce cultural and language barriers in the ambulatory clinic setting.
- Promote culturally safe, trauma-informed practices and cultural humility among staff.
- Collaborate with grant/state partners to align care coordination with program goals.
- Coordinate with external organizations to reduce barriers and improve access to care.
Minimum Job Requirements
Education
- High School Graduate, or
- Certificate of General Educational Development (GED) or High School Equivalency Diploma (HSED), and
- Advanced training beyond High School that includes the completion of an accredited or approved program in Community Health or Completion of program within one year of employment.
Certification / Registration / License
N/A
Experience
- Two years of community health or community service experience.
- One year experience in a clinical setting.
- Strong customer service and interpersonal skills
- Effective written and verbal communication
- Critical thinking and problem-solving
- Discreet handling of sensitive information
- Understanding of cultural humility and trauma-informed care
- Able to work independently and in teams
- Proficient with Microsoft Office
- Knowledge of EHR system
Physical Requirements and Working Conditions
- Clinical office environment with direct patient interaction.
- Occasional travel for home visits.
- Flexible hours may be required to meet patient and clinic needs.
- Ability to sit for most of the workday; occasional lifting up to 10 lbs.
- Operates all equipment necessary to perform job duties.
