We are seeking a dedicated and compassionate Advanced Practice Nurse to join our team. The APN will provide high-quality, patient-centered care in collaboration with our physicians and healthcare team. This role involves assessing, diagnosing, and managing acute and chronic conditions, as well as promoting wellness and disease prevention.

Responsibilities:
- Conduct comprehensive patient assessments and obtain medical histories.
- Order, interpret, and evaluate diagnostic tests.
- Develop and implement individualized patient care plans.
- Prescribe medications and other treatments in accordance with state regulations.
- Perform medication monitoring and prescribe medications as necessary.
- Educate patients and families on health management, prevention, and treatment options and medications.
- Collaborate with physicians, nurses, and other healthcare professionals.
- Maintain accurate and timely medical records.

Qualifications:
- Master’s degree in nursing with a sub-specialty in Family Care
- Valid NJ FNP or APN, RN, CDS, DEA required
- Experience working with children and families
- Excellent communication and interpersonal abilities.
- Strong clinical, diagnostic, and decision-making skills.
