Homeless Prevention Specialist - Berlin ADVOCAP\'s Homeless Prevention
program is accepting applications for a Homeless Prevention Specialist
for our Berlin office. Our Homeless Prevention program is in Fond du
Lac, Green Lake, and Winnebago counties. We assist individuals and
families to overcome homelessness and/or potential homelessness and
poverty by establishing and implementing an individual plan for them to
achieve self-sufficiency, especially housing stability. Responsibilities
Include: Respond to individuals and families in crisis and assist them
in addressing basic needs Work with individuals and families in
establishing their plan for self-sufficiency Coach participants along
their path to self-sufficiency Connect participants to appropriate
community resources and document the individual\'s needs, plans,
services provided, and outcomes achieved Identify gaps in community
services Maintain records documenting participant progress Assess and
enroll participants into appropriate programs Establish trusting
relationship with people \*\*\*\*\* OTHER EXPERIENCE AND QUALIFICATIONS:
Job Requirements: This position will have a bachelor\'s degree in human
services or social services, and 2 years of previous case management
work experience or an equivalent combination is required to meet job
standards. Possess an understanding of poverty and the ability to be
non-judgmental with those served Ability to develop positive,
growth-directed relationships with individuals and families in poverty
Knowledge of and experience in utilizing community resources to help
people in need Acceptable drug screen and comprehensive background check
required Reliable attendance and a professional work ethic \*\*\*\*\*
