- Perform assessments to evaluate patients' functional abilities and develop individualized treatment plans
- Implement therapeutic interventions to improve patients' skills in daily living and work activities
- Educate patients and families on techniques to promote independence and safety in their homes
- Document treatment outcomes and patient progress in accordance with regulatory and company standards
- Collaborate with other healthcare professionals to provide comprehensive patient care
- Stay up-to-date on industry trends and best practices, contributing to the continuous improvement of therapy services
- Master's degree in Occupational Therapy from an accredited program, or Associates degree for COTA
- Current state license to practice as an Occupational Therapist or COTA in Michigan
- Prior experience in a home health or similar setting is preferred
- Strong communication and interpersonal skills
- Ability to prioritize tasks and manage time effectively
- Must have a valid driver's license, auto insurance, and reliable transportation
