You will work closely with patients, families, and interdisciplinary teams to create and implement comprehensive care plans tailored to meet the unique needs of each individual. Responsibilities- Assess patients' health care needs and develop individualized care plans.
- Coordinate patient care with physicians, family members, and other healthcare professionals.
- Ensure compliance with state and federal regulatory requirements in hospice care.
- Monitor patients' symptoms, medication responses, and overall progress.
- Provide education and emotional support to patients and their families.
- Facilitate and attend interdisciplinary team meetings to discuss patient care.
- Document patient care accurately and in a timely manner in accordance with company policies.
- Valid RN license in the state of practice.
- Minimum of two years of nursing experience, preferably in hospice or palliative care.
- Strong assessment and interpersonal skills to effectively communicate with patients and families.
- Ability to work independently and as part of a collaborative team.
- Compassionate and empathetic approach towards patient care.
- Knowledge of hospice regulations and best practices.
- Basic computer skills for documentation and communication purposes.
