Requirements
Your Responsibilities Will Include:
- Conducting assessments of patients' physical and emotional needs
- Must be willing to drive to patient homes
- Implementing and updating personalized care plans
- Providing guidance to patients and their families on end-of-life care and disease processes
- Supervising care provided by team members
- Documenting clinical notes and participating in interdisciplinary meetings
- Current Arizona RN License in good standing
- Minimum of 1 year of hospice experience
- A warm, empathetic approach to patient care
- Willingness to participate in field visits and team meetings
- Adaptability to changing patient needs
