The Part-time Interim Licensed Social Worker assures that the resident’s needs are met in accordance with the mission statement, philosophy, policy and procedures of the facility. Promotes a team concept with other departments and disciplines through demonstrating good interpersonal skills.

Job Description
- Maintains constant interaction with other disciplines in the needs of the residents
- Gather appropriate social information at time of admission
- Maintains on-going contact with family members
- Assists with in-home or in-hospital pre-admission assessments as needed
- Assists residents in psychological, physical, emotional and religious needs
- Assists residents, family members, significant others with Medicaid application redetermination and Medicare issues
- LSW, LCSW or LICSW license required
- Completed internship or fieldwork requirements
- Master's degree preferred
