- Communicates with patients and families after discharge to address concerns and ensure understanding of follow-up care and functional assessment.
- Collaborates with the multidisciplinary team to ensure high-quality patient care.
- Participates in community education as needed.
- Maintains professional relationships with all customers and completes assigned work comprehensively and in a timely manner.
- Provides telephone and face-to-face consultations with patients, answering questions, providing educational information, and reviewing discharge information.
- Effective verbal and written communication skills [Required]
- Ability to read, write and explain moderately complex information to hospital staff with a great deal of attention to detail [Required]
- Ability to communicate effectively in a supportive manner to patients, family, staff and physicians [Required]
- Ability to explain information to patients and families in a way the patient understands, and/or explain departmental or hospital procedures to employees, visitors and patients [Required]
- Ability to observe assess, guide and evaluate others [Required]
Education:
- Associate's of Nursing [Required]
- Bachelor's of Nursing [Preferred]
Field of Study:

Work Experience:
- 2 years critical care and/or emergency care setting [Preferred]
- Previous experience with projects or program start-up [Preferred]
- Previous leadership experience [Preferred]
