Promotes excellent customer services through interactions with care management assistant (CMA) team, patients, physicians, care managers, and the healthcare team. Keeps Care Managers and Care Management Leadership informed of issues or delays impacting timely discharge. Oversees the CMA Coordination of patient care and services arrangements under the direction of a RN or SW Care Manager and in collaboration with the patient/family. Ensures the CMA delivery of regulatory letters to patients or their representatives is in accordance with Federal and State regulations. Prepares reports for Care Management leadership to monitor Care Management processes. Oversees the insurance authorization CMA work for patient post-acute care needs as directed by the Care Manager. Oversees the arrangement of patient transportation and follow up appointments as requested. Monitors CMA staff performance and reports findings to Care Management Leadership.
