Job Overview
We are seeking a dedicated and compassionate Social Worker to join our dynamic healthcare and community services team. Responsibilities

  • Conduct thorough patient assessments to identify social, emotional, behavioral, and environmental needs using clinical research methods and diagnostic evaluation tools such as ICD-10 and ICD-9 coding systems.
  • Develop personalized care plans incorporating evidence-based practices like cognitive behavioral therapy (CBT), psychodynamic therapy, applied behavior analysis (ABA), and crisis intervention strategies.
  • Provide individual, group, and family counseling sessions focused on mental health counseling, behavioral health issues, addiction counseling, PTSD care, Alzheimer's care, autism spectrum disorders, and working with people with disabilities.
  • Facilitate behavioral therapy initiatives and implement applied behavior analysis techniques tailored to children with developmental disabilities or autism.
  • Coordinate with medical professionals to ensure accurate medical documentation while adhering to HIPAA regulations for patient privacy and confidentiality.
  • Manage intake processes and maintain detailed medical records using electronic medical record (EMR) systems; document patient progress accurately in compliance with CPT coding standards.
  • Offer crisis management support during urgent situations by providing immediate intervention and connecting clients with appropriate resources.
  • Engage in case management activities including patient care planning, diagnostic evaluation, data collection for research purposes, and follow-up assessments to monitor progress.
  • Collaborate with multidisciplinary teams in hospital or community settings to deliver holistic care that addresses physical health needs alongside mental health concerns such as psychopharmacology considerations or geriatrics care.
  • Participate in clinical research projects related to behavioral health or social work practices to enhance service quality and effectiveness.
  • Support families through child & family counseling sessions while working closely with educational professionals involved in special education programs.

Skills

  • Extensive knowledge of psychotherapy modalities including cognitive behavioral therapy (CBT), psychodynamic therapy, group therapy, and Christian counseling approaches.
  • Proficiency in motivational interviewing techniques to foster client engagement and motivation for change.
  • Experience working with children—particularly those with autism spectrum disorder—and familiarity with pediatric healthcare environments such as hospitals or clinics.
  • Strong background in home & community care settings combined with expertise in working with individuals with developmental disabilities or disabilities of any kind.
  • Familiarity with EMR systems for efficient documentation; understanding of ICD coding (ICD-10/ICD-9) for accurate diagnosis recording.
  • Ability to perform diagnostic evaluations utilizing medical terminology; experience managing medical records securely under HIPAA guidelines.
  • Competence in crisis management strategies for mental health emergencies involving PTSD or other trauma-related conditions.
  • Knowledge of addiction counseling principles along with skills in applied behavior analysis (ABA) for behavioral modification programs.
  • Experience conducting research studies related to social work or behavioral health practices; adept at data collection and analysis for continuous improvement efforts.
  • Working knowledge of CPT coding used for billing purposes within clinical settings; familiarity with ICD coding standards for diagnoses classification.
  • Ability to work effectively within multidisciplinary teams across hospital or community environments; strong case management skills ensuring comprehensive patient support.
