- Perform patient assessments and develop personalized care plans
- Administer medications and treatments as prescribed
- Monitor patient conditions, communicate changes to the healthcare team, and adjust care plans as needed
- Educate patients and caregivers on disease management, medications, and home care techniques
- Supervise LVN and HHA field staff as needed
- Ensure accurate and timely documentation of patient care
- Participate in on-call scheduling and actively support quality improvement programs
- Registered Nurse (R.N.)licensed in the state of Texas
- Minimum of one year of direct patient care experience(preferably in an acute care setting)
- Strong communication, leadership, and documentation skills
- Compassionate and patient-focused mindset
- Proficiency or experience with OASIS documentation preferred (training available)
- Wound care (including Wound Vacs and Unna Boot applications)
- PICC/Mid-Line and Medi-Port care
- G-Tube, NG Tube, and Tracheostomy care
- Post-surgical wound care and drainage management
- Inotropic medication management
- Dialysis patient medication oversight
